How do I make an appointment?

Appointments can be scheduled Monday through Friday, 8:45 a.m. – 4:45 p.m.

Can I cancel my appointment?

If you need to cancel or reschedule your appointment, please call us at (480) 834-9039. We ask for 24 hours in advance so we may use those times to schedule other patients.

How do I renew my prescription?

We e-prescribe. For the fastest service, contact your pharmacy directly. Please allow at least 48 hours during the business week to accommodate your request, as each request has to be approved by a physician and all physicians are not in the office every day.

If you would like to pick your prescription up at the pharmacy

Please call your preferred pharmacy and follow their instructions for refills. They will contact our office if renewal authorization is needed.

If you would like to fill your prescription by mail order

Please call us at (480) 834-9039, and provide us with the following information:

  • name of the prescription
  • name name and phone number of the mail order pharmacy

How do I pay my bill?

For your convenience, you may now pay your bill online! We accept Visa, Mastercard, Discover and American Express.

How do I contact my physician?

In case of emergency

If you encounter an emergency that is severe and needs attention immediately, either go directly to the closest emergency room, or dial 9-1-1.

After office hours

Desert Kidney Associates provides our patients with 24-hour access to our on-call physician for urgent patient matters. There is always a physician on-call who can be reached through our main number at (480) 834-9039. When our office is closed, the answering service will take your name and number and page the appropriate person. This may not be your usual physician, so you should be prepared to answer some questions for the on-call physician if necessary.

During business hours

Please call our office at (480) 834-9039. Please listen to the menu and choose your physican to be transferred to his or her medical assistant. It is our goal to return all calls in the same business day, but please allow 24 business hours for your call to be returned.

If your issue is urgent, please choose option #1 to speak with the receptionist.

 

How do I request medical records?

To request a copy of your medical records with our paper form, visit the patient forms page, print and complete our Medical Records Release form. Please allow 5-7 business days to accommodate your request.
