Please remember to bring your insurance card(s) and photo identification. If you have already downloaded and completed your forms, please arrive 15 minutes early in order to complete any additional paperwork. If you still need to fill out all of your forms, we recommend you arrive at least 30 minutes before your scheduled appointment time.
Your appointment is scheduled with a board-certified renal specialist. The visit may include a review of symptoms, medical history, medications, blood pressure, kidney-related lab results, and next-step recommendations.
Depending on your referral, the provider may order additional lab testing, imaging, or follow-up visits.
